Incidence of Anesthetic Awareness may be Higher in Low Flow Anesthesia

چکیده

There is a growing interest in applying techniques of low flow anesthesia in different groups of surgical patients. Numerous studies confirm the benefits of low flow anesthesia in comparison to anesthesia with high flow, whereby its limitation and disadvantages have been addressed. The main advantages of low-flow anesthesia techniques involve three areas: pulmonary-anesthesia with low fresh gas flow improves the dynamics of inhaled anesthesia gas, increases mucociliary clearance, maintains body temperature and reduces water loss; economic-reduction of anesthesia gas consumption that results in significant savings of more than 75% and ecological reduction of atmospheric pollution. In addition, the use of these techniques promotes greater understanding of breathing systems and pharmacokinetics of inhalation anesthesia [1,2]. Limitations and disadvantages of this type of anesthesia are: need of modern anesthetic equipment and obligatory gas monitoring, inability to quickly alter inspired concentrations, accumulation of undesirable gases in the respiratory circle, uncertainty about inspired concentrations and faster absorbent exhaustion [3]. Small amounts of inhaled anesthetics in low flow anesthesia (concentrations of the volatile anesthetics may be too low and may cause consciousness during anesthesia) might result in higher incidence of awareness and patients experiencing awareness and recall [4,5].

Background & Objective: Outpatient surgery can be performed by general or spinal anesthesia. Spinal anesthetic drugs have side effects. Many anesthesiologists choose general anesthesia, because of its relative predictability and to avoid undesirable side effects associated with spinal anesthesia. For example lidocaine is frequently associated with Transient Neuroligic Symptoms (TNS). Bupivacain...

Burning Transient Neurologic Syndrome (TNS) which was first described by Schneider et al in 1993, is defined as a transient pain and dysesthesia in waist, buttocks and the lower limbs after spinal anesthesia.1,2 The incidence of TNS after spinal anesthesia with lidocaine is reported to be as high as 10-40%.3,4 This prospective study was designed to determine the incidence of TNS with two differ...

بی‌حسی کامل نخاعی در بیمار مبتلا به آکندروپلازی: گزارش موردی

Background: Total spinal anesthesia is a complication of lumbar epidural anesthesia following undiagnosed subarachnoid or subdural injection of local anesthetic. Although many achondroplastic dwarfs have a normal spine, catheter insertion may be more problematic with a narrow epidural space making a subarachnoid tap more probable.  Other malformations associated with achondroplasia, such as pro...

Detecting awareness in children by using an auditory intervention.

BACKGROUND The incidence of awareness has been reported to be higher in children than in adults. Accurately assessing awareness in children is difficult, and the lack of a specific measure of awareness makes it difficult to determine exactly how many and why children are aware. The aim of this study was to determine the incidence and timing of awareness in children by using auditory stimuli app...

Incidence of awareness with recall during general anesthesia

Background: Experience of awareness and recall during general anesthesia can be very distressing for patients. There is no doubt that surgery pain is the most distressing feature of awareness. This condition can cause neurosis in patients. If patient be aware during surgery and be able to hear some comment, she or he senses weakness, paralysis, feeling of helpless and anxiety, panic and death. ...
